How to: Add a header to Slide Master.
Solution:
Select the 'View' menu and select 'Header and Footer...'.
1) Move to 'Slide Master' view:
a) Select the 'View' menu and select 'Master'. (The Master submenu appears.)
b) Select 'Slide Master'. (The Slide Master slide appears.)
2) Select the 'View' menu and select 'Header and Footer...'. (The Header and Footer dialog box appears.)
3) Select the 'Slide' tab.
4) Select the desired check boxes for the information to include on the slide, and type the desired information in the 'Include on Slide' group.

5) (Optional) Select or clear the 'Don't Show on Title Slide' check box.
6) Do one of the following:
a) Click 'Apply to All', to apply the changes to all the slides in the presentation.
b) Click 'Cancel', to cancel the changes.
NOTE: When in 'Slide Master' view, the header changes MUST be applied to ALL the slides in the presentation.
